Pre-Applications may be picked up from various locations beginning October 1, 2019 (Palatka Housing Authority Main Office, Dr. James A. Long Family Investment Center, Rosa K. Ragsdale Family Investment Center, City Hall(s) in Crescent City, Interlachen and Palatka, Career Source Northeast Florida Community Action, The Heart of Putnam, Putnam County Health Department. Applications may also be obtained on our website palatkaha.org. (print out and complete) then follow instructions. Delivery of pre-applications may be dropped off at the PHA Main Office located at 400 N. 15th Street, Palatka, FL or mailed to: P.O. Box 1277, Palatka, Florida 32178-1277, postmark from October 1st through October 15, 2019 (incomplete or faxed pre applications will not be considered for processing). NOTE: All pre-applications for the Housing Choice Voucher program, submitted to the office or postmarked before or after this date WILL NOT be processed.
